He was predeceased by: his father Sanford Levine. He is survived by: his wife Carol Decker-Levine; his children, Cathy Gillen (Mark), Jay Sanford Levine Jr. (Brenda) and Julie O'Shea (Brian); his stepchildren, Roy Decker (Monica) and Debbie Sterken (Dan); his grandchildren, Colin Zaremba, John, Sofia O'Shea, Kyle, Sam Decker, Brandon and Scotty Sterken; and his great grandchildren, Adrianna and Nathan Zaremba.

In lieu of flowers, the family requests a donation be made in Jay's memory to Wounded Warrior Project or ASPCA.
