He was predeceased by: his parents, Angelo Melendez and Claudette Faye Akers (Burton); and his brother Richard Melendez. He is survived by: his wife Tonya Lynn Melendez of Crossville; his children, Triniti Crockett of Carmi, Taylor Hays of Carmi, Tina Melendez of Chicago, Nick Hays of Crossville, Michael Hays of Carmi and Tiffany Crockett of Norris City; and his siblings, Robert Melendez and Deborah Melendez. He is also survived by eight grandchildren.
