He is survived by: his wife Theresa A. Kamphuis; his children, Sherri Gartman (David), Tom Jr. Kamphuis (Denise), Kenneth Kamphuis (Debra) and Kathleen Perez (Noe); his grandchildren, Heather Bos (Jack), Thomas Calvin (Gricelda), Shaun Gartman, Christopher Gartman (Jailene), Lindsay Smicenski (Mike), Benjamin Kamphuis (Nikki), Noah Perez and Jacob Perez; his great grandchildren, Jackson Bos, Levi Bos, Layla Smicenski, Carter Smicenski and Ares Kamphuis; his sister Carolyn Buntin (Bob); and his sibling-in-law Claire Fondren (Brian).
Visitation will be held on Saturday, October 24th 2026 from 10:00 AM to 12:00 PM at the Colonial Chapel (15525 73rd Ave, Orland Park, IL 60462). A funeral service will be held on Saturday, October 24th 2026 at 12:00 PM at the same location. A burial will be held on Saturday, October 24th 2026 at the Chapel Hill Gardens South (11333 S Central Ave, Oak Lawn, IL 60453).
