He was predeceased by : his grandparents, Otis, Anna Ballard and Allen Fort. He is survived by : his parents, Terry Ballard and Tina Ballard (Fort); his siblings, Kyle Ballard (Jessa Mae) of Streator, Tyler Scott Ballard and Heather Dorton of Ottawa; his grandmother Diane Fort of Minonk; and his uncles, John Corday and Larry Ballard. He is also survived by several aunts, uncles, nieces, nephews, and cousins.
