She was predeceased by: her parents, Dmitro and Anna; and her brothers, Nick, Leon and John.
She is survived by: her husband Milan Jak; and her sons, Mark (John Halm) and Michael Jak. She is also survived by nieces, nephews, extended family members, and dear friends.
