He was predeceased by : his siblings, Shelia, Nancy, Sr., Xavier, Mary, Flor, Jack and Jeremiah.

He is survived by : his wife Catherine; his children, Ulick (Erin), Patrick (Erin), Jerry (Shiho), Maureen Artl (Kevin), Brendan (Kathleen) and Sheila Hoffmann (Kenneth); his grandchildren, Colin, Aidan, Sadie O'Sullivan, Mary, Thomas, Eddie, Seamus O'Sullivan, Akira, Kaela O'Sullivan, Teddy, Cate, Griffin Artl, Rory, Maeve O'Sullivan and Fiona Hoffmann; and his sister Alice O'Sullivan (late Neilly).

Visitation was held on Tuesday, May 14th 2024 from 3:00 PM to 8:00 PM at the Cooney Funeral Home (625 Busse Hwy, Park Ridge, IL 60068). A funeral service was held on Wednesday, May 15th 2024 from 9:30 AM to 10:00 AM at the Immaculate Conception Church (7211 W Talcott Ave, Chicago, IL 60631). A funeral Mass was held on Wednesday, May 15th 2024 at 10:00 AM at the same location. A burial was held on Wednesday, May 15th 2024 at the Maryhill Cemetery (8600 N Milwaukee Ave, Niles, IL 60714).
