He was predeceased by: his father Robert Day. He is survived by: his mother Josephine Day (Wotton); his wife Stacey Day; his children, Jenna and Leo Day; and his brothers, Monty Raymond, John and Paul Day.
In lieu of flowers, the family requests that donations in David's honor be made to Peoria Public Schools Foundation for sports equipment.
