He was predeceased by: his parents, Emil Hawksworth and Helen Hawksworth (Reed); his brother Donald Hawksworth; and his great-niece Brianna Williams. He is survived by: his sister Patricia Butler (Robert) of Metamora Illinois; and his sister-in-law Roberta Hawksworth of Peoria Arizona. He is also survived by several nieces & nephews.

“Jerry Hawksworth lived at the Arcadia Care Morton, IL. I met Jerry he was really nice. I never knew he was from the same church I belong to until the missionaries Elders would come and see me. I'll miss him. But I have a strong Testimony that I will see him again in heaven, there is life on the other side of the veil. There are reunions of family visiting with him. Tracy Franklin”
