She was predeceased by: her parents, Lenzie Warren and Marion Warren (Seither); her husband Sam Hayworth; and her pet Rocky. She is survived by: her children, Angela Hayworth Creech of Pekin and Michael Hayworth (Corinn) of Peoria Heights; her grandchildren, Zakary and Jaxson Hayworth; and her sisters, Judy Carruthers (Bob) of Peoria, Nancy Somers of Peoria and Sally Hogan (Tom) of East Peoria.
Memorial contributions may be directed to Michael Hayworth for an educational fund for her grandchildren.
