He is survived by: his daughters, Deborah Mitchell of Peoria, Barbara Banas (Michael) and Elizabeth Mitchell of Peoria; his grandchildren, Sarah Clemens (Daniel), Kevin Haag (Bridget), Matthew Haag (Haley), Thomas Banas and Nicolette Warne (Brian); and his great grandchildren, Oliver, Maxwell, Alice, Sophie, Daniel, Ryan, Nora, Gracie, Lucy and Nathaniel.
In lieu of flowers, Tom would prefer you send cash directly to him in Heaven but will settle for memorials made to EP!C of Peoria, IL.
