He was predeceased by: his parents, Richard Phelps and Evelan Phelps (Summers); his former wife Kristie Koppen; his stepfather Jerome Liesse; his sister Susan Lynn Phelps; his brother-in-law Donald Kosciewicz; and his in-laws, Erminia, Richard Koppen, Ruth and Donald Johnson.
He is survived by: his wife Debby Johnson of Peru; his children, Alissa Kays (Brandon) of Granville and Neal Phelps (Michael Cunio) of New York, NY; his grandchildren, Beckett and Caroline of Granville; his siblings, Sandra Kosciewicz of Peru and Scott Phelps (Joann) of La Salle; and his sisters-in-law, Donelle Johnson of Milwaukee, WI, Diane Peterson (Charles) of Shelby Township, MI and Dorie Johnson of Tilton, IL. He is also survived by many other family members and friends.
