He was predeceased by: his wife Ruth Cusack; his brother Kevin Cusack of Seattle, Washington.; his parents-in-law, Sam and Rose Portelli; and his parents, Robert Cusack and Irene Cusack (O'Connell).

He is survived by: his children, Deanna Cusack of Fort Myers and Ryan Cusack (Julie) of Oswego, Illinois.; his grandchildren, Logan Porter, Avery Cusack and Emmett Cusack; his siblings, Marilyn Mahon (late John Davis) of Edwardsburg and Robert Cusack (Louise) of Frankfort; his brothers-in-law, Phil Portelli (Lori) of Manhattan and John Portelli (Mardi Hudson) of Lena, Illinois.; and his brother Tom.

In lieu of flowers, Tom would be honored to have memorial donations in his name gifted to Saint Elizabeth Seton Catholic Church in Naperville, with a designation for the Saint Vincent de Paul Society.
