He is survived by: his wife Vicki Boelens; his son Jim Boelens (Jodi); his grandchildren, Anthony DiGvonni, Madison Boelens and Kaylah Boelens (Hunter Shenberger); his brothers, Sam Boelens (Kathy) and Patrick Boelens (Sheila); his niece Ginger Albano (Darren); his nephew John Boelens (Alicia); and his sisters-in-law, Judy Armstrong (Larry) and Kathy Jones (Marv). He is also survived by four great-grandchildren.
