Madison (Robert) of Mound City, IL, Tyrone Cook (Jennifer) of Chicago, IL., Keith Cook of Chicago, IL. and Chaka "Chuck" Cook (Lisa) of Chicago, IL.; her grandchildren, Shaunice Bucker, Shanay Cook, Tyrone Cook, Jr., Tysean Cook, Demetrius White, Latilya White, Brianna White, Brittany White, Donnisha Patterson (Leon), Donnie Blake, Jr., Janiece Blake, Skylar Blake, Shante Cook, Shantrice Cook, Chaka Cook Jr. and Chanelle Burton; and her great grandchildren, Leajah, Leon III, Lejayah, Lenajah, Donnie Jr. III, Jaylen, Brynn, Christian, Elijah, Malachi, Legend Tiearra, Trinity, Serenity, Alexi, Airel, Alyssa, Don Juan, Germanee, Haryu, Tikeyah, Nia-Nisa, Zaina Dalas and Dakota. She is also survived by great-grandchildren, nieces, nephews, cousins, other relatives, and friends.
