He was predeceased by: his father Walter Kozlowski. He is survived by: his parent Laraine Kozlowski (Schmidt); his wife Anna Mirta; his sons, Christopher Kozlowski of San Jose, CA, Neil Kozlowski of Quincy, IL and Peter Kozlowski of Quincy, IL; and his brothers, Walter Kozlowski (Carole) of Hendersonville, TN. and Steven Kozlowski of Hendersonville, TN.. He is also survived by Nieces, nephews, cousins and other relatives.
