He is survived by: his son Jerry W. Jr. "Short" (Michelle); his wife Dunlap of Flat Rock, Illinois; his son Dunlap of Flat Rock, Illinois; his grandson Jaden Dunlap of Flat Rock, Illinois; his brother Gary Treat of Robinson, Illinois; and his siblings-in-law, Marcia Parker of Robinson, Illinois and Danny Eveland of Willow Hill, Illinois.
The family requests memorial contributions, in Jerry's memory, to be made to the Alzheimer's Foundation.
