He was predeceased by: his parents, Andrew Czupka and Jean Czupka (Czosnyka); and his siblings, Ernie Czupka, Giles Czupka, Gerry Bakeris and Diane Barnett.
He is survived by: his wife Candy; his children, Shelly Caldwell (Kevin), Rick Cook (Sandy), Jamie Phelps and Mike Phelps; and his siblings, Rick Czupka (Lois) and Bernie Bowman. He is also survived by 8 grandchildren and 8 great-grandchildren.
