He was predeceased by: his parents, Frank O'Keefe and Myrtle O'Keefe; his wife Carolyn; and his daughter Sharon. He is survived by: his daughter Deb; his son-in-law Bernie; his daughter Cindy; his son-in-law Roger; his sons, Chuck and Tom; and his son-in-law Steve. He is also survived by ten grandchildren and nineteen great grandchildren.
In lieu of flowers, memorial contributions may be made in Dick's name to the O'Keefe family for a memorial to be established later at one of his favorite golf courses.
