He was predeceased by: his parents, John Lewis and Mossie Lewis; and his wife Ruth Lewis. He is survived by: his sons, John (Tamara), Calvin (Kathy) and Jack (Rose); his grandchildren, David, Cierra, Dustin, Derrick, Brandy and Jack Jr.; and his siblings, Larry Lewis, Harmon Lewis, Lois Bryant, Mable Anderson and Mary Rossellott. He is also survived by several nieces, nephews.
