Polly Ann Frazier Obituary
We are sad to announce that on September 4, 2023, at the age of 90, Polly Ann Frazier of Rockford, Illinois passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Charles Jordan and Nell Jordan (Potter); her husband Buddy Frazier; and her sister Jane Blevins. She is survived by: her children, Song, Cathy Schoon (Ray), Jane Stevens (Russ) and Dawn Salzman (Darrin); her grandchildren, Charles Frazier (Dawn), Robert Frazier (Arlyn), Sean Omayas, Katie McWilliams, Tim McWilliams, Jeff McWilliams (Ashlee), Erin McFarland (Mike), Kristin Thornburg (Adam), Jordan Salzman and Ethan Salzman; and her brother-in-law George Blevins. She is also survived by 11 great grandchildren, and 6 great-great grandchildren.
In lieu of flowers, the family requests memorials to Shriners Hospitals for Children.
