We are sad to announce that, on August 27, 2023, at the age of 83, Thomas J. Smith of Rockford, Illinois, born in Sturgeon Bay, Wisconsin passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Roy C. Smith and Julia (Washichek); and his brother-in-law Jay Reis. He is survived by: his wife Jill Smith (Reis) of Rockford, IL; his children, Shelley Smith (Dave Yaniak) and Shawn Smith (Jill) of Frederick, CO; his grandchildren, Sydnee, Parker and Chandler Smith; his brothers, Jim Smith (Heather) and Greg Smith (Mary Ellen); his close friend Pete Carlo (Iva) of Scottsdale, AZ; and his caregivers, Deanna, Karla and Tara. He is also survived by in-laws, and nieces and nephews.
In lieu of flowers, donations in Tom's name to The Michael J. Fox Foundation for Parkinson's Research would be appreciated.
