She was predeceased by: her parents, Joseph Mentlewicz and Johanna Mentlewicz. She is survived by: her husband Michael Pultorak; and her siblings, Katherine Flynn (Patrick), Joseph Mentlewicz (Channel), Thomas (Nancy), Robert (Diane) and Paul Pultorak (Judy). She is also survived by numerous nieces and nephews.
Visitation was held on Wednesday, December 3rd 2025 from 10:00 AM to 11:30 AM at the Anderson Memorial Home & Heartland Cremation Services (21131 W Renwick Rd, Plainfield, IL 60544). A funeral service was held on Wednesday, December 3rd 2025 at 11:30 AM at the same location.
