He was loved and cherished by many people including : his parents, Clifford Forman and Celeste Drye Forman; his spouse Merle Lloyd Shoemaker; his children, Philip Forman (Norma) and Martha Jane Curry; his stepdaughters, Janiece Galloway of Macomb and Leslie Carmack (Mark); his grandchildren, Tim, Rick, Angie, Michael, Brad and Joe Curry; his great grandchildren, McKenze, Madison, Malachi, Layla, Austin, Shelby, Nathan and Mason; his son David Forman; and his former life partner Elizabeth Mae Dace.
