She is survived by: her children, Michael, Deborah, Christopher and Todd; her grandchildren, Michael, Nick, Jack, Brandon, Zachary, Natalie, Ismail, Jacob, Elissa, Karina and Kira; and her siblings, Ann Lentz and Douglas McHenry.
For that reason, Phyllis desired that any donations go to the American Cancer Society.
