He is survived by: his wife Kathy Hazen; his sons, Brandon and Connor; his parents, Jeanene Hazen and Leroy Hazen; his siblings, Jeff (Sandy) and Laura; his nephews, Jared, Nick, Bret and Reid; and his niece Kate.

In lieu of flowers, contributions can be made to St. Jude, a cause deeply important to Scott.
