He is survived by: his parents, James "Jim" Cook and Roberta Cook (Roberts) of Shelbyville, IL; his daughter Alyssa Christine Cook (Jackson Jacobs) of Oakley, IL; his brother Brad Cook (Jessi); his niece Emma Cook of Shelbyville, IL; his nephews, Martin Cook of Mattoon, IL and Logan Cook of Shelbyville, IL.; his pet Raven; and his brother-dog Cooper.
