He was predeceased by: his parents, Claude Smith and Pearl Smith (Lowary); and his sister Carole S. Ulmer. He is survived by: his wife Sue; and his children, Michael D. Smith of Mattoon, IL, Mark D. Smith (Stacey) of Berryville, VA, Matthew D. Smith of Lexington, KY, Sara Sixel (Travis) of Greenville, WI and Beth Massey (Tyson) of Effingham, IL. He is also survived by 17 grandchildren; and 7 great grandchildren.

In lieu of flowers, memorials may be given to the American Heart Association.
