He was predeceased by : his parents, Joseph J. Kotalik and Marie Kotalik; his children, Mary Miller and Robert Kotalik; his granddaughter Beth; and his brother David Kotalik.

He is survived by : his wife Darline L. Kotalik; his sons, Joseph (Elisa) and Lawrence Kotalik (Kathy); his stepsons, Steven, James (Beth), Kevin Hart (Patricia), Brian and Danny Klobnak; his grandchildren, Amy, Joseph, Michael, Julie, Jennifer, Lauren, Theresa and Grace; and his stepgrandchildren, Aidan, Dannelle, Rachel, Michael, Brianna, Veronica and Andrew. He is also survived by his family, especially his grandchildren and great-grandchildren.
