He is survived by: his fiancée Nancy Siders of Springfield; his daughters, Rebecca Polacek (William) of Palos Heights, IL, Sally Kuhn (Ben) of Mackinaw, IL and Jill Hamm (Richie) of Bradenton, FL; his grandchildren, Ryan, Alayna, Will, Claire, Truman, Kayla, Elijah and Nixen; his siblings, Thomas Centko (Karen) of Marquette, MI and Geraldine Miethke of Phoenix, AZ; and his close friend Micky. He is also survived by several nieces, nephews and cousins; and cat.
