She was predeceased by: her parents, Delbert D. Whitehurst and Helen H. Page Whitehurst; her husband Harold L. Jack, Sr.; and her brothers, Delbert Whitehurst and Thomas "Tommy" Whitehurst.

She is survived by: her children, Sheree McKane (Thomas) of Springfield and Harold "Buddy" Jack, Jr. (Stacey) of Springfield; her grandchildren, Jonathon (April) and Bryton; her stepgrandchildren, Courtney, Ariel (Bryant) and Dalton (Aly); her great grandchildren, Joseph and Jonathan; her siblings, Linda Cook (Roger) and Robert "Bobby" Whitehurst (late Linda) of Springfield; her siblings-in-law, Cindy Jack of Peoria, Terry Jack (Rose) of Springfield and Duane Jack; and her cousin Martin "Marty" Ushman of Springfield. She is also survived by several nieces, nephews, cousins and friends.
