He was predeceased by: his father James A. Mayfield, Sr. (Michael). He is survived by: his mother Martha Mayfield (Gerding); and his siblings, James A. Mayfield, Jr. (Beth), Mary Dellert (David), John Mayfield (Keri), Michelle Sorenson (Tim), Anthony Mayfield (Shelley) and Matthew Mayfield (Addie). He is also survived by many nieces, nephews, great-nieces, and great-nephews.
