She is survived by: her husband Keith Cook; her children, Tara Demand (Joe), Lori Baker (Bryson), Michael Cook (Jenny) and Amy Garrett (Alonzo); her grandchildren, Meghan Ellis, Caleb Ellis (Marissa), Josh Ellis (Daeja Ford), Elijah Ellis, Annalisa Segura, Jordan Demand, Christian Demand (Hannah), Zane Baker, Tagg Baker, Cole Taillon, Sadie Starwalt-Cook, Michael Cook Jr., Gage Zitko, Tristan Moore (Bethany), Marcus Latham (Anyssa McCarvey), Marissa Lewis, Alyssa Sexton and Anthony Garrett II; her great grandchildren, Jaylee, Penelope, Keaton and Willow; her stepparents, Sandy Johnson and Rick Myers; her brothers, Steve Johnson (Ardi), Jeff Johnson (Andi), Craig Johnson and Ricky Myers (Megan); her stepsisters, Cathy Bogart (Carrie), Carol LaCoax (Steve Carey) and Cindy Harris (Randy); and her caregivers, Jazmon White and Lisa. She is also survived by nieces, nephews, dear friends.
