He was predeceased by: his parents, Thaddeus J. "Ted" Zmuda and Helen Zmuda (Zychowski); his wife Caryn Rae Chambers; and his brother Jim Zmuda. He is survived by: his children, Rachel BySura (Johnathan) of Lakewood Ranch and David A. Zmuda, Jr. of Vienna, Illinois; his grandchildren, Hayden, Ellie, Sadie, Elizabeth, Nolan, Nathan and Macie; and his sister-in-law Nancy Zmuda of Montana.
