He was predeceased by: his parents, Edward Cook and Rose Cook. He is survived by: his wife Susan Cook; his daughters, Jenny Cook and Laura Van Dien (Michael); his grandchildren, Taylor, Tresten (Ann), Andrea, Scotty, Sawyer and Leo; and his sister Barb Cook. He is also survived by his four great-grandchildren.
In April 2017, Gerry was honored by the Mayor and Waukegan City Council for his distinguished service and lasting contributions to the city he loved.
