He is survived by: his son Joshua Polacek (Brynanna) of Tennessee; his stepson Jason David Embry of Oklahoma City; and his stepsisters, Angie Polacek of Waterloo, Teressa Polacek of Waterloo, Michelle Polacek of Waterloo and Melissa Duran of Guymon.
