She was predeceased by: her parents, Anton Slavicek and Ethel Slavicek. She is survived by: her children, Timothy (Lisa), Philip (Susanne), Jennifer Bristol (Rodney) and Susan Pettyjohn (Michael); and her brother Glenn Slavicek. She is also survived by nieces, nephews, and dear friends.
