He was predeceased by : his wife Arlene; his siblings, Beatrice "Pat" Culhane (late John), John (Kathy), Edward and John T.; his nieces, Patty and Debbie; and his parents, John O'Sullivan and Dora O'Sullivan.

He is survived by : his children, Cathy Evans (George), Peggy Henderson, Paul O'Sullivan (Lisa) and Ann Leifker (Jim); his grandchildren, Emily Evans, Tom Evans, Josh Henderson, Gavin O'Sullivan and Erin Leifker; and his nieces and nephews, John, Tracy and Becky.
