She is survived by: her husband Roy Forth of Xenia; her children, Jody Lee Daughenbaugh (James) of Essex and Daniel Glenn Forth of Highland; her grandchildren, Candra White, Alaina White Aaron Daughenbaugh (Zak Shaffer) and Cody Daughenbaugh; her great grandchildren, Cameron, Jayden, Vanellope, Eathon, Weston and Addison; her sister Helen Gahan of Logansport, Indiana; her nieces, Pam Baker (Rip) of Jacksonville, Theresa Dietrich (Duane) of Logansport, Debbie Forth of Flora and Lori Bowen of Cape Girardeau; her brother-in-law Larry Forth of Flora; her nephews, Kelly Forth and Steve Forth; and her sister-in-law Louise Forth of Flora.

Memorial donations are suggested to Flora First Christian Church, and these will be accepted through the funeral home.
