He is survived by: his son Andrew Middleton (Elesha); his mother Carolyn Stevens; his siblings, Michelle Shumaker, Susan Graham (Kenneth), Patricia Middleton and Brian Middleton; and his stepbrothers, Tom Stevens and Tracy Stevens. He is also survived by 8 grandchildren and 2 great-grandchildren.

In lieu of flowers, the family asks that donations be made to the American Cancer Society.
