He was predeceased by: his parents, Walter Cook and Louella Cook; his wife Myrna Lou McClellan; and his brothers, Don Cook and Dick Cook.
He is survived by: his children, Deb Cook (Susan Gudas) of Ashley, Holly Filbrun (Adeline) of Ashley and Rick Cook of Ashley; his grandson Luke Holsinger of Ashley; and his brothers, Bill Cook (Sharon) and Bob Cook (Bea).
