He was predeceased by : his mother Wendy Mortimore (Smith); his uncles, Jerry "Jake" Cowan, Joe Smith, Steven Smith and David Smith; and his grandparents, Joe Smith and Shirley Smith-Bahls (Cowan).

He is survived by : his father Kris Mortimore; his brothers, Joshua "Blake" Mortimore (Sarah) and Trey Mortimore (Julia Harshbarger); his children, Mariah Mortimore, Owen Mortimore, Bentley Gregory, Kenley Gregory, Kinlee Ingram and Karter Stimac; his niece Eva Mortimore; his children, Whitney Ingram and Jasmine Lusch; his grandparents, Jerry Mortimore and Joyce Mortimore (Simonton); and his aunts and uncles, Mindy Ferguson (Smith), Deann Mortimore, Jerry Mortimore and William Von "Bill" Mortimore. He is also survived by his family, lifelong friends, and his children.

The family asks that in lieu of flowers that memorial contributions be made at any Fountain Trust banking center, "for the benefit of Jakob Mortimore.".
