He is survived by: his wife Lorna Cook of Grove City, OH; his sons, Andy Cook (Rachel) of Columbus, OH and Chad Cook of Columbus, OH; his stepson Brandon McElvain (Kelley); and his brothers, Raymond Cook Jr. of Delaware, OH and Dennis Cook Sr. of Delaware, OH.
