He was predeceased by: his parents, Walter P. Kozubal Sr. and Anna Kozubal (Maxim); his son Paul Edward Kozubal; his son-in-law Dan Jalk; and his siblings, John Kozubal, Joseph Kozubal and Mary Ann Kozubal.
He is survived by: his wife Mary Lou Kozubal; his daughters, Anne Marie Jalk and Eve Marie Fears (Robert); his brother Michael Kozubal (Lynn); his daughter-in-law Donna Kozubal; his grandchildren, Paul Kozubal (Michelle), Julia Fears and Sarah Fears-Carter (Craig); his great grandchildren, Aiden Kozubal and Augustine Carter (Auggie); his cousins, George Krizman, Tony Krizman (Cindy) and Paul Kozubal (Diane); and his caregiver Lisa White. He is also survived by many nieces and nephews.
