He is survived by: his siblings, Janet Mayfield (late David), James Hayes (Debbie) and Margie Myers (Dick); his wife Patricia Hayes (Jones); his children, Anthony Church (Amy) and Leah Simon (Greg); his grandchildren, Abigail Church, Alexander Church (Loren), Rachel Simon and Garrett Simon; his great grandchild Charlie Diana Marie Church; and his caregivers, Rose and Heather of Home Healthcare.
