He was predeceased by: his grandparents, Elmer Lewis, Glen and Margaret Bloyd; and his brother Cory Lewis. He is survived by: his parents, Steven F. Lewis and Sandra Lee Lewis (Bloyd); his children, Alivia Hanson (Tyler) of Madison, IN, Tyler and Luke Lewis of Columbus, IN; his brother Aaron Lewis (Ashlee) of Hoopeston, IL; his granddaughter Adaline Hanson; his grandmother Lou Lewis of Hoopeston, IL; and his nieces and nephews, Allee, Addison, Alex and Axton Lewis.

Memorials may be made to the donor's choice.
