He was predeceased by: his parents, Nicholas J. Costanzi and Sonja Schofield Costanzi; his grandparents, William (Doc), Ethel Schofield, Cal and Jessie Costanzi; and his sister Toni Costanzi.

He is survived by: his sisters, Lisa Ross (Rick) of Columbus and Shannon Smith of Indianapolis; his half-brothers, Mike Costanzi (Jill) of Texas and Tim Costanzi of Illinois; his nieces and nephews, Shelby Ross of Cincinnati, Jenna Smith of Indianapolis, Kyle Ross (Jennifer Harris) of Bargersville, Brody Miller of Bargersville, Nathan Mullins, Damon Estes, Tyler Reynolds of Columbus and Brian Smith of South Dakota; his close friend Mary Clingerman; and also April Watkins.

In keeping with CDC and Indiana guidelines, social distancing must be followed and masks are required.
