He was predeceased by: his parents, Frank Moak and Gertrude Harris Moak; his wife Lori Beth England; and his brother Charles Moak. He is survived by: his children, Alisha Moak of Columbus and Justin Moak of Columbus; his grandchildren, Belicia Garcia, Damienn Martin and Wesley Martin; and his sisters, Linda Moak of Ohio and Sherry Meade (Verdell) of Kentucky. He is also survived by numerous nieces and nephews.
Memorial contributions may be made to the Bartholomew County Humane Society.
