She is survived by: her children, Shannon Hurst (Terry) and Bernard Lee Jones (Lisa); her stepdaughter Tammy Shendler (Kim); her grandchildren, Courtney Hurst (Taylor Johnson), Brayden Jones, Jamison Hurst, Dylan Hurst, Preston Shendler and Doug Workman; her great grandchildren, Ahniyah, Amirah, Kamari, Shyley and Shaydin; her siblings, Burford Lee Forman (Elaine), Clester Forman (Carol), Joe Forman (Cristal), Mary Sue Miller, Cathy Moulder (Eldon), Joanna Estep (Mike), Marie Osborne (Brian) and Connie Forman; and her stepmother Joan Forman.
