He was predeceased by: his parents, Donald Blevens and Mary Manuel Blevens; and his sisters, Dana Blevens and Renee Blevens. He is survived by: his daughters, Ashley Phillippe (Chris) and Amber Blevens (James Trueblood) of Dugger, IN; his brother Michael Blevens (Cristy) of Dugger, IN; and his granddaughter Nila Elizabeth Phillippe.
