He was predeceased by: his father Charles A. Cook, Sr.. He is survived by: his mother Donna Cook of White Pigeon, MI; his wife Gayle Smith; his children, Michael Cook (Dawn) of Elkhart, Charles A. Cook III of Elkhart and Susan Bryan (Richard) of Jones, MI; and his siblings, Tom Cook (JoEllen) of Ft. Meyers, Christine Beachy of Goshen, Cathleen Medford (Doug) of White Pigeon, MI and David Cook of Bristol, IN.. He is also survived by 7 grandchildren and 7 great-grandchildren.
